Instructions
Please read these instructions carefully and fill in as many fields as possible. More information at faith formation at Nativity of Our Lord can be found at https://www.nativityofourlord.net/faith-formation.

		One parent or guardian should complete the form for all family members. This person's information shall be entered under the Head of Household section. Any additional parents/guardians should be entered in the Spouse section, even if the parents/guardians are not married. After completing family information, student registration information is entered. Additional students can be added by selecting the "Add Another Student" button.

		If you would like to receive text messages from faith formation staff regarding programming, please select your provider from the Description drop-down list. Standard text messaging rates apply. All faith formation email notifications will come from notifications@ondemandmail.com. Please add this email address to your email contact list to reduce the likelihood of emails going into Spam.

		CLASS OPTIONS
		This form has two class options for each student. Students must be enrolled in at least one class, but can be enrolled in up to two. The classes are:
		1. Faith Formation Registration 2026-27
		-Any student from 3K - 12th grade who will attend faith formation classes, including Catechesis of the Good Shepherd. Nativity Catholic School students are not required to attend Faith Formation classes outside of school hours, but are welcome to join us if interested.
		2. Sacramental Preparation 2026-27
		-Students who plan to receive a Sacrament in 2026-27, including First Reconciliation, First Communion, and Confirmation. Typically students in 2nd grade will receive First Reconciliation and First Communion. Students in 10th grade will begin preparation for Confirmation, and students in 11th grade will receive Confirmation.

		REGISTRATION FEES: Pre K through Grade 12
		- One Student: $50.00
		- Two Students: $90.00
		- Three or more Students: $110.00

		SACRAMENTAL PREPARATION FEES
		- First Reconciliation/First Communion: $50.00
		- Confirmation: $50.00* (paid in 10th grade)
		*There may be additional retreat fees to cover lodging and food expenses. These fees are paid at the time of retreat registration.

		1. Mail or drop off payment at the parish office. Please be sure to include a memo with payment.

		You will receive an email confirmation after form submission. Faith formation classes are expected to begin in September 2026. Shortly before, you will receive an additional email with your student's schedule and additional required forms.
